Patient Forms

Upon entering our office, you will be greeted and asked to complete new patient forms.  These forms are designed so you can tell us what types of health issue you have.  It is a way for Dr. Zach to determine if chiropractic care can benefit you.  These forms will be available upon entering our office or online.  Your first appointment will take less than 30 minutes.


Next, you will have a consultation with Dr. Zach Winkelmann to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Zach Winkelmann to learn more about you, your condition, and  expectations to determine how chiropractic care can meet your goals.  In the consultation, Dr. Zach will address you specific conditions and first explain the anatomy and what potentially is the underlying cause and second explain how he will help you.  He will also address and questions or concerns you may have.  Dr. Zach’s number one priority is the patient so patient feedback is very important to gaining a full understanding of your conditions.  The consultation is complementary and from this point you can make the decision on whether chiropractic care is the right choice for you. 


After your consultation, Dr. Zach Winkelmann will perform a complete chiropractic analysis.  This will consist of different test depending on your condition.  Reflex, muscle strength, cranial nerves and other test may be performed.  In this portion of the analysis he is looking to see where your subluxations may be.

X-Ray Studies

Depending on your specific condition, Dr. Zach may take X-rays as well.  X-rays are done at the office and can show where your misalignments are located, any contraindications to adjusting and any other issues that are present on X-ray.  Dr. Zach will then briefly review your X-rays with you.  On the second visit, Dr. Zach will further explain your X-rays to you and show you your misalignments or subluxations.

Initial Treatment

After the brief X-ray review, an initial adjustment will be given.  If there are any findings on X-ray that warrant further review, you may be asked to come back the next day for your initial treatment.  The first appointment should take no longer than 30 mins.  At conclusion, Dr. Zach will schedule your next appointment where he will further analyze your X-rays and present the analysis to you.  This is called the Report of Findings.

Report of Findings

Once the information is collected  and examinations are performed, Dr. Zach Winkelmann will give you a detailed report of all findings and answer any questions including:

  1. How can you help me?
  2. How often do I need to come in?
  3. What  will  my treatment  cost?

After reviewing your health history,  goals, and examining your spine and X-rays, Dr. Zach Winkelmann will discuss recommendations and notify you if  your condition  requires care with other providers.   Dr. Zach Winkelmann  will  provide the best  treatment and  wellness program for your needs.


After the report of findings you will know what conditions you have and how long treatment will take.  It is important to know that chiropractic care is a treatment program.  What you can expect after the initial treatment:  Some people will feel immediate relief, however, the relief may only last a couple days.  Some may feel sore, which is your body responding to the treatment.  Some may even feel worse, which means your body is adjusting to the changes and is a good sign.  The body’s natural physiology takes time to heal.  For example, if you break a bone you will be casted for 6-8 weeks and then still be sore.  The reason this happens is that is how long it takes your body to heal.  The same goes for chiropractic care.  Chiropractors are moving bones and soft tissue will move as well. The body takes time to adjust to these changes. 

Preventative Care Program

After you complete your initial treatment period, Dr. Zach will recommend a Prior to leaving, the doctor will suggest a preventative care program.  This will consist of adjustments that will occur once a week, once every two weeks or even once a month.  The program is tailored to each individual and what conditions they have.  Everyone heals and reacts to chiropractic are differently.  This is why the preventative program will be customized for every person.  The purpose  of our preventative care program is for  you  to achieve good spinal alignment, have a  healthy diet, exercise, and maintain a positive mental state.
